Congratulations to the Como Park Falcon Heights Living at Home Block Nurse Program!!!! We are patting ourselves on the back for the gracious grant received from the Stevens Square Foundation. This grant will be used to identify and work with elders in their homes on a bi-weekly basis to foster exercise and healthy, safe living environments.
