We know that the right help, information or service when you or your family needs it can be key to managing your day-to-day needs. The following list of resources may help you find what you need.

Como Park/Falcon Heights Living at Home Block Nurse Program also provides many of these services.  Depending on the service, fees may apply.  Please contact us for more information at 651-642-1127.

Provides information about assisted living services, home care and nursing homes.  Assistance in choosing a nursing home.  Handle complaints about nursing homes and home health care service providers. ​

​Pre-Admission Screening / Alternative Care, Ramsey County

County staff will help individuals 65 and older evaluate options that would prolong their ability to live safely in their community setting rather than a nursing home. Persons of frail health, with low/modest incomes, can receive these services at no cost or a cost set by their ability to pay.  Some Medical Assistance eligible, disabled individuals under 65, may also qualify for assistance.

Social Services provides protection and support services for children and adults who are vulnerable or who need mental or chemical health services. Adult Services include Adult Protection, Disability Services, and Mental and Chemical Health services. ​
