We know that the right help, information or service when you or your family needs it can be key to managing your day-to-day needs. The following list of resources may help you find what you need.

Como Park/Falcon Heights Living at Home Block Nurse Program also provides many of these services.  Depending on the service, fees may apply.  Please contact us for more information at 651-642-1127.

Long Distance Medical Transportation Services

​Only for trips OVER 200 Miles in distance. This group is a specialist in non-emergency long distance medical transportation services.  You can trust Long Distance Medical Transportation Services to get your patient or loved one to their destination safely.

​Fairview Transportation

​Specific to Fairview & HealthEast Medical Appointments, Clients: Fairview Senior Transportation makes rides available to those eligible for services. All riders must be age 55 or older. Riders must be temporary disabled and unable to drive themselves. Rides cost $5 each way (prices subject to change). Eligible riders can receive up to 6 rides per calendar year. Appointments that include any type of anesthesia or sedation do not qualify for this service.​​

​​Specific to HIV clients. Formerly Rainbow Health Initiative. Support with transportation to medical appointments or pharmacy visits. We also have volunteer drivers who can assist with food shelf pick–ups and deliveries on a limited basis. We offer bus passes for eligible clients who cannot get a bus pass from other programs.

MTM is a healthcare and transportation management company whose mission is to partner with clients in developing innovative solutions for accessing healthcare, increasing independence, and connecting community resources in the most cost-effective manner.

​​Transportation services are available for getting to and from medical appointments, pharmacies, support group meetings, and meal programs.  Clients receive either a bus card or transportation by staff in an Agency vechicle.  SAYFSM will provide a voucher for a taxi ride only when a client is too ill to take public transportation.
